In partnership with KSLA News 12 and Capital One Bank, the Volunteer Center at United Way of Northwest Louisiana is proud to announce a new program that will recognize outstanding individuals throughout the ArkLaTex that contribute to their communities by volunteering. Beginning in January 2010, every month a volunteer will be selected as the Volunteer of the Month. They will be featured in a story on KSLA News 12 to acknowledge their dedication to community service.
At the end of the year, all twelve recipients of the Volunteer of the Month Recognition will be celebrated at a Volunteer of the Month banquet sponsored by Capital One Bank.
Please download the nomination form provided below and return to United Way of Northwest Louisiana by fax at 318-698-4035 or by e-mail at volunteers@unitedwaynwla.org or by mail at 402 Edwards Street, Shreveport, LA 71101.
Nominations are accepted on a rolling basis throughout the year.
Anyone who is committed to volunteering and community service can be nominated! Kids and groups can be nominated too!
Employers, teachers, friends, family, non-profit organizations, companies, and government entities can all nominate someone for the recognition.
Selected nominees will be notified by phone. We will not be able to acknowledge receipt of nomination forms or notify nominees who are not selected.
